Using this questionnaire
You should only use this questionnaire if you have been given an access code by the relevant NHS service or organisation.
Please provide information that is accurate to the best of your knowledge. You can review and change your answers before submitting.
Not an emergency service
This questionnaire is not monitored as an emergency or crisis service and should not be used to request urgent help.
If you are experiencing a mental health crisis, call NHS 111 and select option 2. If there is immediate risk of serious harm, call 999 or go to your nearest A&E.
Availability and changes
The service may be updated, paused or withdrawn to maintain security, accuracy or availability.
Submitting the questionnaire does not guarantee a specific outcome. The service team will review your information and contact you about appropriate next steps.
