What this service is for
Leeds Confed Autism Questionnaire is a confidential digital questionnaire for adults who have been asked to share information about autism-related experiences, support needs and possible assessment referral needs.
Your answers help Leeds GP Confederation understand how your experiences affect daily life and what support, adjustments or next steps may be appropriate.
How it works
If you have been given an access code, you can complete the questionnaire online. You can save progress and return later if you need more time.
After submission, a PDF report is generated and shared with the service team so they can review your answers and contact you about the next stage.
