The questionnaire collects the answers you provide, including information about your support needs, autism-related experiences, health, appointment preferences and contact details.
Technical information may also be recorded to operate the service securely, such as submission time, audit logs and email delivery logs.
How your information is used
Your information is used by Leeds GP Confederation to review your questionnaire, understand your needs and contact you about support, advice or referral next steps.
A PDF report of your answers may be emailed to you and to the service team. Your information is not used for marketing.
How your information is protected
Questionnaire responses are handled as confidential health information and stored securely. Access is limited to people who need it to provide or administer the service.
